WHO NEEDS TO COMPLETE PATIENT REGISTRATION?

Everyone who is seen at Power of Wellness for any reason needs to have a completed patient registration packet on file. Whether you are wanting to establish primary care at POWHS, need an urgent care/same day visit, or simply need some labs drawn without a provider visit the same process will apply. Registration packet, identification, insurance (if applicable) are all required for any patient that we see in clinic. Filling them out ahead of time can save time and stress at the time of your appointment.

CALL

Call us to schedule your appointment and ask any questions you have. We will gather some basic information over the phone so that we can create your chart and put you on the schedule.

COMPLETE

Complete the registration packet prior to your arrival. You can bring it with you, email it in, or fax it back. If you do not have your packet completed when you arrive, we may need to reschedule you.

CHECK-IN

Check-in at least 15 minutes prior to your appointment time. We will need to get a copy of your ID and insurance card (if you have one) - so be sure to have those with you. Digital copies can be emailed to us when you check-in, or before hand.

CHECK-OUT

Check-out at the front desk after you appointment. This is when we can schedule any follow-up appointments, get you access to your patient portal, and collect any remaining balance for the visit/services provided.

2 / COMPLETE REGISTRATION

We have one form that works for everyone; beyond that there are some visit-specific forms that may also apply to you.

If you are making an appointment for someone under 18, we will need the Minor Consent signed by the parent/guardian. We cannot see the patient otherwise.
If you are having a CDL, USCG Phys., Workers Comp. or other specific exam we need you to have the appropriate forms filled out in advance. They can be sent in with registration.

them using the form below. If you'd prefer, you can always bring the completed forms to

your appointment with you.
